Need a copy of your Form 1095-C?
Sprinklr no longer mails paper copies of Form 1095-C automatically.
Form 1095-C is a health coverage statement that shows the medical coverage Sprinklr offered you during the year. You do not need to file this form with your tax return unless it is requested.
Due to a change in federal law aimed at reducing paperwork, employers are no longer required to automatically print and mail paper copies of Form 1095-C. As a result, Sprinklr does not mail this form by default.
2025 Form 1095-C will be available by March 2, 2026. Current employees can access it electronically in Workday by selecting:
Menu > Personal > Benefits > Tasks > My ACA Forms
If you would like a copy of your Form 1095-C for your records, you may request one at any time by contacting the Benefits team at benefits@sprinklr.com. A copy will be provided upon request.
